What to Expect for the First Visit

At the initial intake appointment, Dr. Bae will meet with you to discuss presenting concerns, your child's needs, goals, and to collect background information. If applicable, it is helpful to send relevant medical records, previous evaluation reports, and school records for review. This session will help Dr. Bae to formulate her initial impressions, for you and your family to get a better sense of the practice and Dr. Bae, and to develop a plan moving forward.

Please read and complete the new patient paperwork, as well as send relevant records, prior to the initial appointment. Dr. Bae will review practice policies and answer any questions you may have at the first appointment.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical/mental health care will cost.

Under the law, health care providers need to give patients who do not have insurance or who are not using insurance an estimate of the expected charges for medical services. You have the right to receive a Good Faith Estimate of the total expected cost of any non-emergency services, including testing and psychotherapy services. You can ask your health care provider for a Good Faith Estimate before you schedule any service. The Good Faith Estimate shows the costs of services/items that are reasonably expected for your healthcare needs. The estimate is based on information at the time the estimate is created. It does not include any unknown/unexpected costs that may arise during the course of your care. You may be charged more if special circumstances occur. In non-emergency circumstances, you will be provided with an updated Good Faith Estimate for any new expected charges. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. You may contact the provider to let them know the billed charges are higher than the Good Faith Estimate and ask them to update the bill to match the Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.
